Clearances

All individuals involved with our youth programs must complete the following items. Click each link to complete. Email all documents to Registrar@PENNathleticsClub.com. When you receive the PA Child Abuse History Clearance later, it's fine to email it separately.

PA Child Abuse History Clearance

  • Click "CREATE INDIVIDUAL ACCOUNT"
  • Click "Next"
  • Complete the online form.

You will receive an email notification once the Clearance is complete. When you do, log back in, download the PDF document and submit via email.

PATCH (Pa Access To Criminal History)

  • Click "New Record Check"
  • Click check box, then "Accept"
  • Complete the online form.
  • Click to download the PDF document and submit via email.

Disclosure Statement

Complete this if you have lived in Pennsylvania for the last 10 consecutive years.

  • Print
  • Sign
  • Take pictures of the signed pages and submit via email.

FBI Criminal Background Check (fingerprint based)

Complete this if you have not lived in Pennsylvania for the last 10 consecutive years. There is a $27 charge for this, but you will be reimbursed by the club.

  • Click "Schedule or Manage Appointment."
  • Complete the online form.
  • Schedule fingerprinting.
  • Once you receive the results in the mail, take a picture and submit via email.
